The three assent models suggest different reading and communication levels, not university age cutoffs. The IRB determines assent requirements and documentation for the actual children; consider maturity, comprehension, language, disability accommodations, and current Sac State instructions.

Blank Cayuse writing worksheet: A guided blank worksheet using the four locally verified Cayuse headings, with prompts for every major part of the study.
Parent or guardian permission: Blank permission language with separate choices for the core study, an optional interview, and optional recording.
Younger child assent spoken script: A spoken model for younger children or children who need a read-aloud explanation; choose by comprehension, not age alone.
Child assent short written version: A short reading-level model, often useful for elementary-age readers, with space for questions and separate choices.
Teen assent more detailed version: A more detailed assent model for adolescent readers, including privacy, withdrawal, and optional recording.
Adult survey information and consent: Adapt for educators, parents answering about their own experiences, or other adults; verify anonymous versus coded collection.
Adult interview consent: For colleague, parent/caregiver, educator, or other adult interviews; separate the interview decision from optional audio recording.
Research invitation email or letter: A neutral invitation adaptable for adult surveys/interviews or for families considering child research.
Site cooperation and permission letter: A request and unsigned response outline for the school or organization; separate from IRB approval and participant consent.
Data management and storage plan: A practical worksheet for institutional storage, separate permissions/key/data folders, access, retention, and disclosure review.
